About the “Deciding Together” Decision Aid
Our LVAD Decision Aid was made through the collaboration of an interdisciplinary team and is funded through the generous support of The Patient Centered Outcomes Research Institute (PCORI).
This award allowed us to speak to LVAD patients, caregivers, and clinicians to figure out what patients want to know when making decisions about LVAD therapy. In all, we completed over 80 patient, caregiver, and clinician interviews to inform the creation of this decision aid.

About this site and the Heart Mate 3 Risk Score
The creation of this website from our “Deciding Together” decision aid and its integration with the Heart Mate 3 Risk Score was made possible by the Agency for Healthcare Research and Quality (grant number R01HS027784).
The Heart Mate 3 Risk Score was developed and validated from the MOMENTUM 3 trial population to predict mortality for patients undergoing implantation with the Heart Mate 3 LVAD. To read more, click here. The Heart Mate 3 Risk Score validation studies, team, and funding are separate from that of the patient decision aid.
